CONTRACT for Reiki Sessions with Scot Albert

I the undersigned;

Do hereby agree to prepay to Scot Albert on a monthly basis the sum of
$400.00 /month for the purpose receiving one session per week of Reiki.
(This fee covers a standard month period.)

(Reiki is a form of energetic healing that 
falls under the category of laying on of hands 
that has its roots in the culture of Japan.)

I understand these sessions will be on the 
same predetermined day and time of the week.

Please choose your time and make sure you note it on your appointment calendar.

If you, the client, should miss a previously agreed-upon
appointment, you simply forfeit that particular session, without receiving a
refund for the amount prepaid for that session.  

This agreement is to last for the duration of;

I further understand these sessions are not covered by any  
form of insurance nor are they to be construed as medical treatment.
